Care planning
Every resident has an individual care plan which is designed to meet their specific needs. This is the contract of care between the home, the resident and their family or friends and gives staff the right information to enable them to meet residents' precise needs, wants and aspirations.

We encourage relatives and friends to get involved in care planning as their insight and information can help us provide the best tailored care possible. For example, if a resident is living with dementia, it is really helpful for family or friends to provide a full history of their life. This enables us to understand the person and their likes and dislikes, and can help us deliver personalised care which is right for them.
We regularly review our residents’ care plans with the residents themselves and with their relatives to ensure we keep delivering the best possible care. Any changes to the resident’s needs or preferences that may have taken place are incorporated into the care plan, allowing us to continually tailor the care we provide to the individual receiving it.
